The ESG-CV Rapid Re-Housing (RRH) Case Manager (CM) is a grant-funded position working with individuals experiencing homelessness who are at high-risk for coronavirus due to health conditions. The ESG-CV RRH CM will case plan with clients referred by the Northeast Florida Continuum of Care Coordinated Entry to quickly obtain, maintain and sustain permanent housing. This case manager will provide ongoing case management, crisis intervention, and resources and referrals, while maintaining complete and accurate client records supportive of the mission and vision of Mission House, Inc.

The ESG-CV RRH CM is aware of emotional and practical needs of clients and staff members in all interactions, offering positive and constructive feedback regarding quality of work and effort of clients and coworkers. The employee adjusts priorities and schedules to meet specific client, program and agency needs. Reports and/or corrects problems related to safety, cleanliness and comfort of resident and work areas. Universal Precautions and Infection Control techniques are followed. Understands own role facility safety plans and procedures related to fire, weather and other disasters is demonstrated.

Essential duties and responsibilities

• Conduct new client intakes to assess needs, identifying immediate barriers and options. • Meet with assigned clients in person at least monthly, with weekly/daily phone contact, to provide case management coordination of services for immediate needs such as medical, legal, substance abuse and mental health; obtaining identification documents; transportation assistance; employment, education, or income assistance/budgeting; and connection to resources, all to facilitate permanent housing stability. • Assist clients in identifying and obtaining permanent housing, including transporting clients to locate and/or view prospective housing, communicating with prospective landlords/property management companies, assisting with move-in of donated furniture, etc. • Engage in frequent, ongoing case management with housed clients to monitor and support ongoing housing stability. • Utilize and submit CoC/Mission House forms and documentation to obtain financial assistance for clients. • Have knowledge of and utilize Evidence-Based Practices in service delivery, such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First. • Enter complete case notes and services in ClientTrack Homeless Management Information System (HMIS) and utilize Mission House Case Management forms for submission to Program Director to complete monthly reports. • Collaborate with Mission House Case Management team, other Mission House programs, and partner agencies, as needed, to ensure client needs are met. • Participate in conferences, workshops, special projects and staff meetings, as assigned. • Represent Mission House professionally and assist during Mission House events, as directed. • Ability to lift up to 30 pounds. • Perform all other duties, as assigned.


• Must have personal car and valid Florida driver’s license. • Must have acceptable annual Motor Vehicle Record check. • Must adhere to required insurance coverage levels (Bodily Injury & Property Damage Liability: $100,000 each person; $100,000 each accident), if driving a personal vehicle for work-related purposes. • Must pass a Level II background check.

Education and Experience

• Bachelor’s Degree in a helping profession, and at least one year of work experience in Social Services field required. • Must have knowledge of basic case management principles and best practices. • Must have knowledge of housing, community resources and substance abuse/mental health issues, interventions and treatment-planning. • Ability to manage and de-escalate crisis situations required. • Excellent written and verbal communication skills required. • Must be proficient with computer use and computer applications.
