Nassau County Council on Aging, Inc. seeks a Fall Prevention Case Manager for the 1901 Island Walk Way Fernandina Beach office. This a full-time, Monday-Friday position with normal hours of 8:00am – 5:00pm.  Occasionally, the hours may vary slightly, and some after hours and weekend hours could be required. 

The Fall Prevention Case Manager (FPCM) will visit seniors in their home, assessing and educating on fall prevention. Candidates must be knowledgeable about the array of community-based services and resources available to address the needs of seniors and their caregivers.  Candidates should also have knowledge of obstacles facing the aging and vulnerable population.

Essential duties and responsibilities

• Visit seniors in their home, assessing and educating on fall prevention • Increase awareness and access to NCCOA and other community services and resources to meet individual needs • Complete an assessment, care plan, as well as arrange for and follow-up on services and resources • Maintain individual case records • Build an expanded support network and resources with community partners in the community


• Excellent verbal communication skills. • Excellent writing, computer skills – must be proficient in Microsoft office including word, excel, power point and publisher. • Self-motivated and self-directed skills. • Must be proficient in online meeting platforms such as Zoom, Webex, etc. • Ability to create webinars. • Community outreach skills. • Use of personal vehicle for outreach and home visiting purposes countywide. • A valid driver’s license. • Must be able to pass the Level 2 background check required by DOEA and must adhere to Drug Free Workplace standards.

Education and Experience

• Bachelor’s degree in social work, psychology, sociology, nursing, gerontology or related field. • Experience in adult education through online and classroom platforms. • Two (2) years case management experience. • Experience working with the elderly or other vulnerable population in a customer focused environment.

Salary/Salary Range

$30,000-$35,000 per annum
