On behalf of the Non Congregate Shelter Program, Changing Homelessness, Inc. is posting a contract position with an expected employment period of no less than six months.  This is a temporary contract position with a pay rate of $20 per hour, 40 hours per week. The COVID-19 Non Congregate Shelter program is a multi-agency effort to provide shelter for homeless individuals in response to the COVID-19 public health emergency.

The Isolation Facility Coordinator will partner with the Isolation Facility Rapid Rehousing Case Managers and the Client Case Coordinators (CNA’s) to provide services and support to individuals housed in the Non Congregate Shelter.

The population served will include homeless asymptomatic high-risk individuals needing social distancing as a precautionary measure, such as people over 65 or with certain underlying health conditions (respiratory, compromised immunities, chronic disease). In addition, it may include homeless individuals who test positive for COVID-19 who do not require hospitalization but need isolation (including those exiting from hospitals) or those who have been exposed to COVID-19 who do not require hospitalization. Selected individuals will have the necessary Personal Protective Equipment to manage individuals isolated with COVID-19 test pending and/or test positive and medically stable.

Essential duties and responsibilities

Daily tasks include, but are not limited to: • Monitor need for isolation or quarantine and work with local public health, emergency medical, and other local service providers to ensure health and safety for clients and staff. • Arrange non-emergency ambulance transportation for clients entering Isolation Facility. • Maintain relationships with food delivery services/agencies. • Coordinate food drop offs and pick-ups, ensuring procured food is stored properly and sorted as needed. • Coordinate with Rapid Rehousing Case Managers to ensure daily client check-ins. • Resolve conflicts between guests & community/hotel • Ensure client information is recorded in HMIS, under COVID-19 Isolation Facility Project • Provide support if unusual or emergency situations should arise. • Meet with Isolation Facility Program Manager, Rapid Rehousing Case Managers, and the Client Case Coordinators, as needed. • Provide daily client updates


• Shelter intake and management expertise • HMIS database management input and reporting • Conflict Resolution • Successfully pass Level 2 background check

Education and Experience

Past experience with homelessness preferred
