The COVID-19 Non Congregate Shelter program is a multi-agency effort to provide shelter for homeless individuals in response to the COVID-19 public health emergency.
The Isolation Facility Rapid Rehousing Case Manager will provide virtual and onsite client support with weekly check-ins and housing case management for Non-Congregate shelter residents placed in hotels in response to COVID-19 isolation and social distancing precautionary measures.
The population served will include homeless asymptomatic high-risk individuals needing social distancing as a precautionary measure, such as people over 65 or with certain underlying health conditions (respiratory, compromised immunities, chronic disease). In addition, it may include homeless individuals who test positive for COVID-19 who do not require hospitalization but need isolation (including those exiting from hospitals) or those who have been exposed to COVID-19 who do not require hospitalization.
The Isolation Facility Rapid Rehousing Case Manager will work closely with the Isolation Facility Coordinator and Program Manager to ensure the client needs are met. Selected individuals will have the necessary Personal Protective Equipment to manage individuals isolated with COVID-19 test pending and/or test positive and medically stable.
Essential duties and responsibilities
• Conduct interviews with clients to evaluate their needs, coordinate and connect client to medical, mental health, housing, education, transportation, and employment resources. • Work with the client to create a housing stability plan (using S.M.A.R.T. goals) and monitor household progress and accomplishments to reach identified goals to help them exit to a permanent housing destination. • Assist clients with housing identification, leasing, and move-in assistance. • Perform housing inspections (process may include virtual inspection) • Provide weekly client check-ins and support. • Submit requests for Temporary Financial Assistance on behalf of the client in compliance with funding requirements. • Input data and provide Client updates in the Homeless Management Information System (HMIS). • Meet with Isolation Facility Coordinator and Isolation Facility Program Manager, as needed.
