Essential duties and responsibilities
• Facilitate evidence-based interventions to the targeted population (older adults-50+). • Attend ongoing program meetings at all times. • Model professional, responsible behavior at all times. • Maintain accurate, complete records of programs and projects (including sign in sheets, group/individual dosage forms and NOMS used to provide monthly status reports. • Conduct prevention intervention groups to target population • Identify community resources to benefit program, agency and participants. • Conduct HIV Rapid Tests, counseling and referrals • Utilize database to record program attendance and demographics. • Assist with the development of documents and reports. • Organize and provide presentations to community agencies businesses on prevention services • Uphold the overall mission and goals of the program. • Provide training to program staff and other support consistent with job roles assigned. • Assist with developing marketing tools and recruitment and retention of program participants. • Act as program lead when out in community by implementing tasks and duties as assigned by Supervisor. • Coordinator and conduct group and individual intervention groups sessions with emphasis on behavioral health, HIV/AIDS and addiction needs for at risk older adults.
• The ability to appear for work on time. • The ability to follow directions from a supervisor. • The ability to facilitate group sessions and develop presentations relevant to the targeted population. • Ability to accurately and articulately communicate Community Rehabilitation Center (CRC) and Ready To Respond services to community members, community organizations and lead officials throughout city. • Ability to manage multiple tasks and deadlines proficiently. • Adhere to programmatic and company work rules and procedures.
Education and Experience
• Bachelor’s degree and five or more years in providing Substance Abuse (SA) or HIV/AIDS prevention services. • Preferred requirements include a Master of Public Health or master’s degree in social science, health, or MBA with three or more years in SA and HIV/AIDS prevention intervention. • Must possess strong presentation skills and communicate effectively in front of large groups.
