Description
Family Promise of Jacksonville is one of five agencies serving families with children at-risk or experiencing homelessness. It seeks a case manager for its shelter program for families with children experiencing homelessness and to develop post-care program for graduates of both the shelter and its diversion program.
Essential duties and responsibilities
Clinical/Guest-Specific Activities Engage in necessary case management activities, including screening of Guests, intake, shelter orientation, family goals and plan development, barriers to goals, case planning meetings, review of progress towards goals for affordable housing, sustainable employment, financial awareness, education & childcare, medical needs, and social services. Assist guests in applying for benefits, resume assistance, medical/dental needs, school enrollment/issues, transportation issues, bus schedules and alleviating barriers. • Facilitate hosting 3 families in the temporary shelter (Congregational rotation). • Make referrals to appropriate resources such as childcare, workforce preparation, affordable housing and other needs and provide assistance in accessing those resources. • Develops relationships within the community as resources for families. • Coordinates shelter and meals with the host congregations. • Review Guest progress/obtain updates from Guests on a daily basis. • Create forms as needed. • Maintain guest demographics - admissions, discharges, days in shelter, etc. • Update and revise guest guidelines as needed. • Establish good working relationships with guest adults. • Manage guest disputes. • Monitor daily activities of guests. • Take guests to appointments (assistance needed, last minute opportunity, etc.) • Report and document guest-volunteer disputes. • Exit planning for guests including deposits, utilities, furniture, and a stability plan. • Provide follow-up for former guests (Post Care Program) • Develop and manage a post care program including workshops and learning opportunities for former guests and guests.. • Maintain HMIS and other records with information for case management, services provided and data collection for grants. Administrative • Answer phones and FPJ doors. • Return phone calls within 24 hours. • Ensure the office is covered during working hours. • Identify families with stories to share. • Take photos of donations and families with permission. • Working hours 9am-6pm. Family Resource Center • Oversee Guest activities. • Ensure guest responsibility for day-to-day upkeep and cleanliness of Family Center (lounge, guest rooms, and guest restrooms) including trash removal. Miscellaneous • Share on-call duties with the Director. • Oversee Office Volunteers in the absence of the Director. • Drive a van as needed. • Perform other tasks assigned by the Executive Director.
Qualifications
Knowledge, Skill and Abilities: • Ability to work effectively with a wide range of people. • Skilled at providing guidance and encouragement that motivates people to better their lives. • Knowledge of benefits and available services in the Jacksonville area. • Good writing and communication skills. • Ability to multitask effectively. • Excellent documentation skills. • Necessary computer skills. • Must be available weekdays in person. Some weekends when needed.
Education and Experience
The successful candidate will have: • Case management experience, a minimum of 2 years • Valid Florida driver’s license. • Bachelor’s degree with major course work in the areas of psychology, social work or a related human service field with some social service experience preferred or a minimum of two years experience with family homelessness in a social service setting.
Salary/Salary Range
$15.00 - $24.00 per hour Health savings account
