To provide case management supportive services to famillies served by Family Promise of Jacksonville, Inc. at the Family Day Center.
Essential duties and responsibilities
*Engage in necessary case management activities Including screening guests, intake toward goals, assisting guests in case plan meetings, review of progress toward goals, assisting guests in applying for benefits, resume assistance, medical/dental needs, school enrollment/issues, transportation issues, bus schedules, etc. *Make referrals to appropriate resources (childcare, workforce preparation, affordable housing, etc.) *Develops relationships within the community as resources for families. *Maintain guest demographics *Update/Revise guest guidelines as needed. *Establish good working relationships with guest adults. *Discharge planning for guests (deposits, utilities, furniture, etc.) *Assist with Family Support Program, providing case management.
Valid Florida driver's license *Flexible schedule *Ability to work effectively with a wide range of people. *Skilled at providing guidance and encouragement that motivates people to better their lives. *Knowledge of benefits and available services in the Jacksonville area. *Good writing and communication skills. *Great documentation skills *Necessary computer skills
Education and Experience
Minimum of two years experince with family homelessness in a social service settiing. Associate of Arts degree in related field Bachelor of Arts degree in related field preferred.
