Job Description

Manages the health care activities for the agency’s health clinic and residential program by performing the following duties:

Essential duties and responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned. Works a flexible schedule, as needed, to meet the needs of participants, facility, and staff. Coordinates health care services of five (5) group homes, personal supports and the health clinic, to include assisting in the development of program policies and procedures. Be accessible after normal work hours & for consultation to staff regarding chronic, acute, and emergency health concerns for group home residents, as necessary. Assists in pre-screening, admission, and discharge activities, as needed, for program participants that may require the assistance of nursing care. Prepares or assists in preparation of medical assessments, outcome development processes, and preventative/restorative medical care plans of program participants. Functions as a liaison between each group home resident’s physician and residential staff. Coordinates health support services of participants receiving medical, nursing, or therapy services from contracted health care entities. Be familiar with each participant that requires nursing care as to make daily recommendations or adjustments regarding care, as indicated by their status. Work in conjunction with the TESS Director, Residential Director, & Group Home Managers to ensure a proper balance of health care services and training services for each participant. Develop, implement, and/or review participant’s health care plan to maintain their highest possible degree of functioning and self-care, as prescribed by their physician’s orders or support plan. Oversee the administration, storage of medications, and records for compliance with applicable program standards. Provide Medication Training and Validation of Medication Administration to employees, per the APD 65-G guidelines Attend Support Plan meetings, as necessary. Assist with the coordination of staff training, including orientation, regarding health and safety. Assist the Development department to create measurable metrics, report outcomes, and contribute to grant proposals related to the Health Care Program Assist the health clinic nurse as necessary. SUPERVISORY RESPONSIBILITIES: Supervises the nurse that provides the daily health care activities in the agency’s Health Clinic.

Education and Experience

EDUCATION and/or EXPERIENCE: RN Certification and two years nursing experience. LANGUAGE SKILLS: Ability to read, analyze, and interpret common scientific and technical journals, medical and psychological diagnostic reports, financial reports, and legal documents. Ability to respond to common inquiries or complaints from consumers and families, referral sources, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors. MATHEMATICAL SKILLS: Ability to work with mathematics for budgeting, billing, and program evaluation statistics. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret and extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. CERTIFICATES, LICENSES, REGISTRATIONS: State of Florida licensure as a Registered Nurse (RN) Valid Florida Driver’s in good standing is required and must be maintained during employment. Certification in First Aid and CPR or persons seeking such certification are preferred. OTHER SKILLS and ABILITIES: Knowledge of theories and practices of personal, vocational, and behavioral counseling. Knowledge of medical and psychological aspects of rehabilitation. Familiarity with Support Plan process. Familiarity with professional ethics related to working with persons with disabilities. Ability to market program services to the general public or referral agency personnel. Knowledge of standards of Agency for Persons with Disabilities. Salary/Salary Range

Pay: $50,000.00 - $51,995.00 per year
