Case Manager – This position is a part-time billet. Requires 15-20 hours a week. Candidate will perform intake with participants entering the shelter. Assist clients in creating a plan and monitoring them to ensure goals are met. Case Manager will split hours between 2 locations with flex hours 8:00am-8:00pm
Essential duties and responsibilities
• Keep records or prepare reports for management concerning clients. • Provide clients with referrals to outside services as needed and facilitate their progress. • Interview individuals to compile information on social, educational, criminal, institutional, or drug history. • Plan and implement activities for target clients which support the goal of economic self-sufficiency. • Submit reports and review reports or problems with supervisor. • Provide direct service and support to clients, such as conducting a needs evaluation, or resolving complaints. • Order Military Records • Assist clients with resolving barriers • Maintain confidentiality files • Provide training and classes as needed • Conduct comprehensive client assessments to determine eligibility. • Develop trusting relationships which encourage and motivate clients to achieve personal, financial and behavioral goals while respecting relationship boundaries. • Conduct regularly scheduled one to one sessions to establish goals and monitor progress. • Create and reassess clients’ progress regularly and modify service plans as necessary. • Coordinate services provided by partner agencies. • Maintains detailed spreadsheets related to client eligibility for reporting purposes. • Perform all other duties as assigned • Comply with federal, state, and company policies, procedures, and regulations. • Researching and advocating for appropriate public assistance resources for clients • Ensuring that all case files, and other records, strictly comply with policies, regulations, and procedures • Assist other staff members as needed to ensure efficiency of the overall organization.

Education and Experience
Associates degree or equivalent work experience • Minimum of 2 years of social service experience • Successfully pass Level 2 background check and drug screening
