Description
The Senior Care Coordinator is involved in all aspects of the BEAM Senior Service Program, which includes coordination of services both within and outside of BEAM.
Essential duties and responsibilities
• Conduct interviews with potential clients to identify needs and determine eligibility for the program. • Identify and provide clients with referrals to outside resources and services as needed. • Maintain support of assigned clients, monitor service plans, and adjust if necessary. • Address client concerns, questions or special needs as needed. • Create and update program service documentation and processes as needed. • Collect program data and assist with required reporting. • Collaborate with Program Manager, volunteers and other staff to ensure smooth program operation. • Assist with BEAM special events or initiatives, especially those related to senior clients and/or program volunteers Working Conditions • Must have a valid Driver’s License and access to personal transportation. • Must be able to work on occasional nights and weekends. • Must be able to work in a fast-paced environment. • This position requires regular, in-person attendance at BEAM locations. At the discretion of the manager, a percentage of the time spent in this position may be performed remotely within the Jacksonville area. Physical Requirements • Must have ability to sit for extended periods of time. • Must have the stamina to use a computer for extended periods of time. • Must have the ability to lift materials up to 50 lbs. • Must tolerate some bending, stooping, and squatting. • Ability to use keyboard unassisted. • Ability to use telephone unassisted. • Must be able to stand for up to 2 hours in various temperatures.

Education and Experience
• Bachelor’s degree, preferably social science or related field. • 1-3 years of experience working with underserved individuals, preferably seniors.
Salary/Salary Range
$35,000- $ 38,000 / year
