Job Title: Aftercare Case Manager (MHOP)

Hours: Full-Time

The primary role of the Aftercare Case Manager is to provide case management to those with mental illness, substance abuse and/or co-occurring disorders who are enrolled in the mental health jail diversion program, the Mental Health Offenders Program (MHOP). The Aftercare Case Manager will promote independence and quality of life through the coordination of appropriate services and the provision of on-going support as needed by the consumer. The role of the Aftercare Case Manager is important in coordinating required and needed services from across service systems (i.e., mental health, criminal justice, developmental services, and addictions). Case managers fulfill a vital function for consumers by working with them to realize personal recovery goals.

Essential duties and responsibilities

SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities. ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Interacts and provides case management services utilizing a recovery oriented, patient centered model 2. Conduct weekly home visits with all MHOP Aftercare clients 3. Outreach to clients wherever required 4. Provides accurate information about available assistance and completes necessary referrals 5. Refers clients to appropriate programs for assistance 6. Enter data into Client Track and Athena (EHR) 7. Works collaboratively with MHOP team to complete required reports in order to document program performance outcome measures in Excel 8. Work closely with the closely with the Correctional system, Behavioral Health, Medical and Housing Program staff in determining the clients’ needs in order to achieve their goals 9. Assist clients in building social skills in the community that will enhance positive relationships 10. Attend staff development trainings and meetings as directed 11. All other duties as assigned


• Ability to establish cooperative working relationships with clients, colleagues, community partners, and volunteers • Ability to interact with people from diverse cultural and racial backgrounds • Excellent customer service skills • Excellent interpersonal skills, leadership skills, and ability to work independently in a fast-paced environment as a member of a team • Ability to manage and de-escalate crisis situations • Past experience working with mentally ill clients • Working knowledge of community resources • Good computer skills needed • Valid Driver’s License and ability to travel • Acceptable annual Motor Vehicle Record check • Adhere to required insurance coverage levels (Bodily Injury & Property Damage Liability: $100,000 each person; $100,000 each accident) if driving a personal vehicle for work-related purposes • Ability to pass a Level II Background Screening

Education and Experience

• A Bachelor’s Degree in Human Services • Able to work a flexible schedule including weekends • At least 2 years experience as a case manager or equivalent experience
