Job Title: Rapid Re-housing and Diversion Case Manager
Department: Human Services
Reports To: Rapid Rehousing Program Manager
FLSA Status: Exempt
Hours: Full Time

The role of the Rapid Re-housing and Diversion Case Manager is to provide financial assistance to divert clients from entering the homeless system when eligible, or to rapidly re-house clients within 30 days that qualify for ESG assistance. Additionally, the Rapid Re-housing and Diversion Case Manager, while providing temporary financial assistance, will also assist clients in developing income streams, either through employment development, or disability processing, in an effort to ensure long-term sustainability.

Resident, Guest and Internal Customer Focus:
The Rapid Re-housing and Diversion Case Manager is sensitive to emotional, spiritual and practical needs of resident, families, guests and staff members in all interactions. Provides positive feedback and comments about the quality of work and effort that have been offered by residents, families and coworkers. Adjusts priorities and schedules to meet specific customer needs. Reports and/or corrects problems related to safety, cleanliness and comfort of resident and work areas. Universal Precautions and Infection Control techniques are followed. Understands own role facility safety plans and procedures related to fire, weather and other disasters is demonstrated.

Essential duties and responsibilities

Supervisory Responsibilities: None Essential Duties and Responsibilities: 1. Provide financial assistance for up to 5 months for clients that qualify for rapid re-housing. 2. Provide initial financial assistance for clients that qualify for diversion. 3. Assess program qualification criteria of client and identify immediate housing options. 4. Provide monthly follow up for diversion and rapid re-housing clients. 5. Inform Case Managers about diversion and rapid re-housing requirements. 6. Participate in conferences, workshops, special projects and staff meetings. 7. All other duties as assigned.


• Knowledgeable of substance abuse/mental health issues, interventions and treatment planning. • Knowledgeable of housing resources. • Basic computer skills. • Basic knowledge of community resources. • Ability to manage crisis situations. • Personal car and valid Florida driver’s license • Acceptable annual Motor Vehicle Record check • Adhere to required insurance coverage levels (Bodily Injury & Property Damage Liability: $100,000 each person; $100,000 each accident) if driving a personal vehicle for work-related purposes • Ability to pass a Level II Background Screening

Education and Experience

Education and/or Experience: • Bachelor’s Degree in a helping profession, or 5 years of experience working in Social Services. • A minimum of 1 year experience working in rapid re-housing is preferred.
