Job Title: North Florida Rapid Re-housing Case Manager
Department: Human Services
Reports To: Housing Specialist/Rapid Re-housing Program Coordinator
FLSA Status: Exempt
Hours: Full Time

The case manager works directly with the families coming from coordinated intake by developing goal plans to meet their immediate housing needs, providing crisis intervention and resource referrals. Maintains computerized case management and hard copy files.

Essential duties and responsibilities

1. Provide clients with move in costs and monthly rental support as needed (max 12 months) 2. Assess program qualification criteria of client and identify immediate housing options 3. Assisting families in planning of housing attainment and sustainability, including job training, education, money management income, and benefits attachment 4. Monitors progress on housing stability plan 5. Provide ongoing support, crisis intervention and housing stability support 6. Coordinate housing, benefits, medical, legal, substance abuse services as needed 7. Keep client files current in case file and database 8. Participate in conferences, workshops, special projects and staff meetings 9. All other duties as assigned Resident, Guest and Internal Customer Focus: The North Florida Rapid Re-housing Case Manager is sensitive to emotional, spiritual and practical needs of resident, families, guests and staff members in all interactions. Provides positive feedback and comments about the quality of work and effort that have been offered by residents, families and coworkers. Adjusts priorities and schedules to meet specific customer needs. Reports and/or corrects problems related to safety, cleanliness and comfort of resident and work areas. Universal Precautions and Infection Control techniques are followed. Understands own role facility safety plans and procedures related to fire, weather and other disasters is demonstrated.


• Bachelor’s Degree in a helping profession and 2 years’ experience working in Social Services • A minimum of 1 year experience working in rapid re-housing is preferred

Education and Experience

• Knowledgeable of substance abuse/mental health issues, interventions and treatment planning • Knowledgeable of housing resources • Basic computer skills • Basic knowledge of community resources • Ability to manage crisis situations • Personal car and valid Florida driver’s license • Acceptable annual Motor Vehicle Record check • Adhere to required insurance coverage levels (Bodily Injury & Property Damage Liability: $100,000 each person; $100,000 each accident) if driving a personal vehicle for work-related purposes • Ability to pass a Level II Background Screening
