UF Health St. Johns is the lead agency for the St. Johns Continuum of Care, managing the coordinated processes of housing resources as well as being an access point for individuals and families facing a housing crisis in our community. As the social service program, Care Connect utilizes a standardized assessment and prioritization process to assist households in linkage with the most appropriate housing resources for their needs and goals, as well as additional supports to assist in sustainability and overall wellbeing in every aspect of their life. Each household is assessed for prioritization and vulnerability, then matched with an appropriate housing crisis resource within the St. Johns Continuum of Care and receives resource navigation and support until all needs are met.

The Housing Team Lead serves as the primary leadership for all Housing Community Health Associates, programmatic coordinator of Care Connect permanent supportive and rapid rehousing projects, and is responsible for compiling and analyzing monthly reporting to ensure those served are receiving the care they need. The Team Lead functions as an advocate and support to clients and staff alike through facilitation of biweekly provider meetings and internal team meetings.

As a member of the Care Connect Leadership Team, the Housing Team Lead leverages data to drive process and program improvement to ensure all individuals and families served receive the same level of services. When opportunities for improvement are identified, the Housing Team Lead serves as the facilitator of education and growth for team members, and continuing program evaluation.

Essential duties and responsibilities

-Serves as Care Connect Housing Program primary leadership for all Care Connect Housing Programming and associated staff, to include: Coordinated Entry, Care Connect + Rapid Rehousing Program, Care Connect Permanent Supportive Housing Program- Patriot Place. -Responsible for initial training and continued coaching and development through routine 1:1s, for all Housing Community Health Associates. -Responsible for creating and monitoring reporting to ensure Housing programs provide quality case management services to all clients consistent with Care Connect core practices and meet ESG grant requirements. -As a member of the Care Connect Leadership Team, provides weekly operational reporting, and recommendations, in the following areas: case load distribution, staffing model effectiveness and provider performance. -Responsible for the management of the coordinated entry housing crisis process, including documentation of clients in need of housing assistance, facilitating collaboration with housing service agencies for assessment and referral, and ongoing evaluation and revision of the housing process. -Completes required agency, funding, legal, statistical and fiscal reporting documentation and submits to appropriate stakeholders in an accurate and timely matter. -Facilitate weekly and biweekly case conferencing of identified households for resource matching within the coordinated entry housing crisis process, as well as conduct the referral process within the Care Connect Database to community providers. -Coordination and oversight of Care Connect+ permanent supportive housing projects, including but not limited to, resident eligibility and intake into project, as well as assistance with ongoing supportive services to residents. -Responsible for overseeing all aspects of Emergency Solutions Grant Funding and Built for Zero Flexible Funding, in collaboration with the UF Health St. Johns Foundation -Meet with individuals and families to define and set goals that are client centered. Document interventions regularly in Care Connect Database for the clinical record and outcome reporting. -Develop care plan and interventions with client input, monitor compliance with housing plan and problem solve barriers to client sustainability. Provide support for client issues, resource needs, and refer to community partners as appropriate -All other duties as assigned by department

Qualifications

Level 2 DCF Background Check required upon hire.

Education and Experience

Education Psychology or Related Area of Study Experience 1-year Program Coordination 1-year Supervising Case Workers, Interns or Students
