JaxCareConnect is designed to efficiently connect uninsured neighbors in Duval County to primary care options. The program was designed and initially piloted by the Duval Safety Net Collaborative (DSNC), a local network of free and charitable clinics and federally qualified healthcare centers. LIFT JAX is an initiative of business and civic leaders with a mission of eradicating generational poverty through neighborhood revitalization. Access to Healthcare is a key component within their Community Wellness pillar, with key focus on increasing the percentage of residents with a medical home. LIFT JAX and JaxCareConnect have aligned together to ensure greater access to healthcare in Jacksonville’s Historic Eastside through a shared Health Advocacy Navigator (HAN). This position will utilize JaxCareConnect’s successful operational model within a concentrated geographic footprint of the Historic Eastside community. The HAN will be integral to the success of this project and will be the first point of contact for clients seeking to be connected to a medical home. The HAN will work individually with each patient to guide them through the clinic’s eligibility process, examine and help overcome barriers to achieving optimal health, aid with public health applications they may qualify for, and provide ongoing support along their healthcare journey.

Essential duties and responsibilities

The Health Advocacy Navigator (HAN) will report to the JaxCareConnect Administrator, in partnership with LIFT JAX. The HAN will be co-located within key Historic Eastside neighborhood locations and the JaxCareConnect Office. The primary focus of this role will be to support residents in accessing primary care and navigating other community resources to ensure optimal health outcomes. A successful HAN will build working relationships with nearby safety net clinic staff, solve access problems, and support residents in navigating the free and charitable clinic system. The HAN helps residents understand their rights and responsibilities concerning the medical care delivered within the safety net clinics and the healthcare system. The HAN will also meet one-on-one, as needed, with residents to verify current eligibility for services, evaluate current insurability options, and provide social determinants of health screenings. The HAN will assist patients in accessing social services needed to improve their health outcomes. Due to the focus on the Historic Eastside neighborhood, this position will spend a percentage of their time expanding outreach and engagement by working with local neighborhood-based partners. This is an exempt position and may require some non-traditional work hours i.e., evening and weekend shifts. Job Responsibilities and Duties: • Reports to the JaxCareConnect Administrator. • Work within LIFT JAX’s neighborhood-based mission and approach. • Coordinate with staff within the Duval Safety Net Collaborative (DSNC) clinics to review patient eligibility, schedule appointments for new patients, and address patient’s needs. • Meet with patients to explain eligibility, most appropriate primary care options, and assess barriers such as transportation, childcare, etc. by utilizing a standardized social determinants of health assessment tool. • Conduct intake screening (Social Determinants of Health assessment) for clients, when applicable. • Evaluate patients’ qualifications for insurance, including UF Health – City Contract, Medicaid, and Affordable Care Act Insurance Plans, and support patients through the application process i.e., required documents. • Assist patients in locating affordable prescriptions through completing applications for patient assistance programs (PAPs) of major pharmaceutical companies, accessing prescription discount programs and locating charitable pharmacies locally. • Based on self-sufficiency assessment, evaluate barriers to improving access to healthcare, medication compliance, etc. and manage referrals to appropriate social services. • Adhere to the program’s operating policy, procedures and guidelines when addressing a client’s health care needs. • Manage and maintain a caseload of diverse clients with a vast variety of health and social needs, often complex situations. • Targeted outreach through working alongside Eastside neighborhood partners to engage residents and further identify local barriers to access. • Participate in weekly team meetings and other events where staff participation is required.


• Excellent written and verbal customer service skills. • Capability to maintain and respect confidentiality and HIPAA guidelines. • Ability to work independently and on a team. • Organizational, decision-making, and problem-solving abilities. • Excellent technology skills and comfort level working with various databases, applications and data input systems.

Education and Experience

• College degree (Associates or above) or equivalent work experience. • Experience with patient advocacy in the health care industry preferred. • Experience with UF Health City Contract and Medicaid applications preferred. • Experience with case management, following up on referrals and managing multiple clients at a time. • Excel, Word, and PowerPoint skills required. • Electronic Medical Records experience is a plus. • Experience as Affordable Care Act Navigator a plus. Additional Position Information: • Fulltime / Exempt Position • Starting Salary: $40,000 annually • Benefits include health (85% premium covered) and basic life insurance (100% premium covered) for the employee, with vision, dental, and other support available for employees and dependents • Paid vacation, sick leave, and fifteen (15) paid holidays per year, including two “floating” holidays to be used at employee discretion with approval from employee’s supervisor • Reliable personal transportation and home office with high-speed internet required • Must live within or at a commutable distance to Duval County, FL. Although Manager may partially work from home, this is NOT a remote position and local travel is required • Ability to work some evening and weekend outreach / event opportunities • Ability to complete work projects and meet timely program operation, meeting, or funding needs, which may occasionally require flexibility in scheduling evenings / weekends We Care Jacksonville, Inc. Salary/Salary Range

$40,000 - $43,000
