Description
We are seeking a compassionate and resourceful Client Navigator/Case Manager to provide comprehensive, client-centered support for women veterans. The primary focus of this role is to support women veterans residing in our transitional home and oversee the operations of our food pantry. In addition, you will connect clients with essential resources, counseling, and services to address their physical, emotional, social, and financial needs. You will work closely with each client to assess their unique circumstances, develop personalized care plans, and coordinate with service providers and community partners to ensure progress toward achieving their goals. This role works between two office locations and occasional local travel.
Essential duties and responsibilities
The primary focus of this position is residents of the transitional housing community and food pantry. Responsibilities include but are not limited to: •Must be flexible with schedule. •Minimal nights and weekends (emergency client calls). •Conducting intake assessments and creating individualized service plans for clients. •Coordinating and facilitating access to social services, healthcare, housing, employment, education, and veteran and other community resources. •Develop individualized service plans (ISPs) in collaboration with clients tailored to each resident’s goals. •Conduct regular check-ins to track progress and address challenges. •Coordinate services such as counseling, job training, or financial literacy programs. •Schedule house finding trips. •Conduct Weekly room and home inspections. •Advocate for clients and connect them with appropriate resources. •Transport clients to appointments and house hunting trips as needed. •Keeping a record of individuals that reside in the transitional home. (enter dates, exit date etc.) •Ensures adherence to transitional housing program guidelines and safety standards; facilitates resident support services and collaborates with social •service providers to promote residents' progress toward stable, long-term housing.
Qualifications
•Successfully pass Level 2 background check and drug screening • Valid Florida State Driver's License • Strong interpersonal and communication skills • Skill in organizing resources and establishing priorities • Working knowledge of community resources • Skilled at building trust and rapport with people from diverse backgrounds • Ability to work alone on own initiative, often with minimum supervision, as well as part of a small team • Ability to communicate effectively, both orally and in writing • Ability to foster a cooperative work environment • Skilled in the use of personal computers, including knowledge of Microsoft Office • Ability to work with a diverse team in a fast-paced environment • Enthusiasm and the ability to thrive in an atmosphere of constant change • Ability to maintain the confidentiality of identifying client information
Education and Experience
Minimum Position Requirements: • Associates degree or equivalent work experience • Minimum of 2 years of social service experience
Salary/Salary Range
$37,000-$44,000
