Beaches Emergency Assistance Ministries (BEAM) is dedicated to supporting individuals and families in the Beaches community, with a focus on providing emergency assistance, promoting stability, and fostering long-term well-being. Through our Senior Program, we work to ensure that older adults in our community can age in place safely and independently.
The Care Coordinator for BEAM’s Senior Program will play a key role in supporting senior clients who face financial, social, and logistical challenges in the Beaches community. This role will involve case management, coordinating services such as pantry deliveries, friendly phone calls, and minor home repairs, as well as ensuring seniors have access to transportation and housing assistance. The Care Coordinator will provide personalized support, working directly with clients and their families to ensure they are linked to the appropriate services, enabling them to maintain independence and enhance their overall well-being.
Essential duties and responsibilities
Duties and Responsibilities • Develop, implement, and monitor individualized service plans for senior clients based on their specific needs, preferences, and circumstances. • Manage a caseload of senior clients, ensuring they receive timely and appropriate services • Regularly reassess service plans and provide ongoing case management to track client progress, ensuring services remain aligned with evolving needs. • Conduct home visits to assess clients’ living situations and identify additional needs. • Facilitate access to BEAM’s food pantries and delivery services, ensuring seniors receive regular food assistance and nutritional support. • Assist clients with scheduling minor home repairs to ensure safe living conditions. • Assist in securing transportation for essential errands • Serve as a point of contact for seniors and their families, guiding how to access BEAM’s programs and other local services. • Educate clients and families about available community resources and provide referrals to additional support services when needed. • Ensure seniors’ voices are heard and respected while coordinating with various providers to meet their needs. • Address client concerns, questions, or special needs as needed. • Maintain accurate, confidential records of client interactions, service plans, and progress updates in Salesforce. • Create and update program service documentation and processes as needed. • Assist in preparing reports for funders, grants, and other stakeholders, ensuring compliance with reporting requirements, gathering data for grant applications, and demonstrating the program's success. • Collect program data and assist with required reporting. • Collaborate with other staff members, volunteers, and community partners to ensure clients are receiving comprehensive support. • Collaborate with Program Manager, volunteers, and other staff to ensure smooth program operation. • Assist the Program Manager with planning, organizing, and executing special initiatives, events, and outreach activities related to the Senior Program • Other duties as assigned
Qualifications
Preferred Qualifications • Commitment to the mission of the organization. • Bachelor’s degree, preferably in social science or related field. • Knowledge of community resources available to seniors • Experience working with or compassion for underserved individuals, especially seniors. • Strong organizational skills and the ability to manage a caseload and multiple priorities with attention to detail and flexibility with changing tasks. • Strong communication and interpersonal skills, verbal and written, with a client-centered approach. • Knowledge of Microsoft Office Suite applications. • Strong cultural competence: ability to build positive relationships with a wide variety of stakeholders. Working Culture • Must have a valid Driver’s License and access to personal transportation. • Local travel is required for client visits, community events, and meetings with service providers. • Must be able to work on occasional nights and weekends. • Must be able to work in a fast-paced environment. • Primarily office-based with occasional home visits, community outreach, and meetings with partner organizations. • This position requires regular, in-person attendance at BEAM locations. At the discretion of the manager, a percentage of the time spent in this position may be performed remotely within the Jacksonville area. Physical Requirements • Must have ability to sit for extended periods of time. • Must have the stamina to use a computer for extended periods of time. • Must have the ability to lift materials up to 50 lbs. • Must tolerate some bending, stooping, and squatting. • Ability to use keyboard and telephone unassisted. • Must be able to stand for up to 2 hours in various temperatures.
Salary/Salary Range
$17 - $20 / hour
