JOB SUMMARY: The Caregiver Support Navigator will serve as a key liaison between our organization and the broader healthcare community of Nassau County, FL. The Caregiver Support Navigator is responsible for building, maintaining, and nurturing relationships with older adults, caregivers, families, community professionals, and other community support resources. The Caregiver Support Navigator serves as a knowledgeable guide, helping caregivers and older adults navigate available resources and supports.

Essential duties and responsibilities

• Provide personalized information and referrals to community resources, such as respite care, support groups, home health care, transportation, and financial assistance. • Educate caregivers on available services, eligibility requirements, and application processes. • Conduct comprehensive needs assessments for caregivers and older adults to identify challenges and service gaps. • Offer emotional support and guidance to caregivers, addressing stress, burnout, and self-care strategies. • Establish and maintain strong relationships with clinicians, hospital personnel, treatment providers, discharge planners, care navigators and other referral partners. • Track client and member interactions and maintain accurate, confidential case records. • Assist with the organization and facilitation of caregiver workshops, support groups, and informational sessions. • Advocate for the needs of caregivers and older adults within the organization and the broader community. • Identify and pursue new outreach opportunities to grow the referral network. • Represent Nassau County Council on Aging with the highest degree of integrity and professionalism. • Work closely with Advocate Home Healthcare, The Club Adult Day Health Center, and Caregiver Support teams to align outreach strategies with program capabilities. • Collaborate with Advocate Home Healthcare, The Club Adult Day Health Center and Caregiver Support managers and staff to grow enrollment. • Support the Adult Day Health Center /Caregiver Support services programs as needed. Collaborate with the ADHC Manager in order to achieve desired outcomes. • Remain informed and knowledgeable on current caregiver strategies, data, information, and trends. • Other duties as assigned.

Qualifications

• Strong understanding and knowledge of: o Home healthcare, associated systems and payer sources. o Community services and resources available to older adults. o Home & community based services. o Alzheimer’s disease and other forms of dementia. o Caregiver needs and resources. • Excellent interpersonal, communication, and public speaking skills. • Empathy, patience, and cultural sensitivity when working with diverse populations. • Ability to manage multiple cases and maintain organized records. • Ability to travel locally or regionally as needed. • Ability to multi-task. • Self-motivated, goal-oriented, and comfortable working independently. • Proficient in Microsoft Office. • CPR and First Aid Certified.

Education and Experience

WORK EXPERIENCE / REQUIREMENTS: • 2+ years of experience working with caregivers, older adults, aging services, care navigation, case management, or caregiver support. • Strong knowledge of community resources for older adults and caregivers. • Reliable transportation and valid driver’s license. • Must be able to pass the Level 2 background check required by DOEA. • Must adhere to Drug Free Workplace standards. EDUCATION: • Bachelor’s degree in social work, gerontology, human services, or a related field.

Salary/Salary Range

$48,000 per year
