Coordinates the health care activities for the Residential Program and the agency’s Health Clinic
Essential duties and responsibilities
include the following and other duties as assigned: Works in conjunction with the Director of Residential Services, the Director of Training, Education, and Support Services (TESS), and Pine Castle’s Group Home Managers to ensure a proper balance of health care services and training services is provided for each participant. Coordinates the health care services of Pine Castle’s group homes, personal support trailer, and the Adult Day Training (ADT) health clinic, to include assisting in the development of program policies and procedures. Works a flexible schedule, as needed, to meet the needs of participants, facility, and staff. Is accessible after normal work hours & for consultation to staff regarding chronic, acute, and emergency health concerns for group home residents, as necessary. Assists in pre-screening, admission, and discharge activities, as needed, for program participants that may require the assistance of nursing care. Prepares or assists in preparation of medical assessments, outcome development processes, and preventative/restorative medical care plans of program participants. Conducts health screening to ensure familiarity with the health care needs of all program participants as to make daily recommendations or adjustments regarding care. Coordinates health support services of participants receiving medical, nursing, or therapy services from contracted health care entities, to include functioning as a liaison between each group home resident’s physician and the group home staff. Serves as a First Responder to all emergency incidents within the ADT Program to determine and initiate appropriate actions necessary to ensure the wellbeing of individuals involved. Responsible for participant’s health care plans to maintain their highest possible degree of functioning and self-care, as prescribed by their physician’s orders or Support Plan, and attends Support Plan meetings, as necessary. Oversees the proper storage and administration of participant’s medications, as well as maintains records for compliance with applicable program standards. Provides Medication Training and Validation of Medication Administration to employees, per the Agency for Persons with Disabilities 65-G guidelines. Assists with training, including orientation, regarding health and safety. Assists the Development Department to create measurable metrics, report outcomes, and contribute to grant proposals related to the Health Care/Nurse Program. Prepares other documentation and reports as required.
Qualifications
Knowledge of theories and practices of personal, vocational, and behavioral counseling. Knowledge of medical and psychological aspects of rehabilitation. Familiarity with the Support Plan process. Familiarity with professional ethics related to working with persons with disabilities. Ability to market program services to the general public and referral agency personnel. Knowledge of standards of Agency for Persons with Disabilities.
Education and Experience
RN Certification and two years nursing experience. Valid Florida Driver’s in good standing is required and must be maintained during employment. Certification in First Aid and CPR or persons seeking such certification are preferred.
Salary/Salary Range
$28.57-$31.57
