Purpose of Position:
This position is responsible for the development, growth, and success of the 988/211/Mission United (MU) network of social and mental health related service organizations and serves as the face of 988/211/MU in our communities. The Community Partnerships Manager plays a critical role in curating and maintaining the community database used by 211 I&R Specialists, Mission United’s Veteran Care Coordinators, Crisis & 988, and the broader community for information and referral purposes, particularly related to basic needs, mental wellbeing support and veteran specific services. The Community Partnerships Manager analyzes the needs of our communities and collaborates with service organization partners to meet the social and wellbeing needs of our community. This position collaborates internally to ensure adherence with our accreditation standards and is responsible for the collection, coding, organization, and maintenance of resource data. The Community Partnerships Manager will lead our continuous quality improvement network strategy, monitoring network performance, addressing gaps in our continuum and implementing solutions, to increase the performance of the network.
Essential duties and responsibilities
Key Responsibilities and Essential Functions: • Represent UWNEFL at community events with the goal of increasing community awareness about 988, 211, and MU • Develop strong working relationships with our network partners • Participate in community collaboration to address unmet needs through innovation and partnership • Leverage technology and implement mechanisms for providing performance feedback to our network partners from community members served • Maintain a current, comprehensive, computerized inventory of appropriate nonprofit, charitable and government organizations in an assigned service area structured in accordance to policy and procedure • Update agency profiles in the database at regular intervals, but at least annually • Review current provider agency information for completeness and accuracy, correct formatting, grammatical errors, and duplicate entries prior to conducting annual survey • Prospect new resources for inclusion in the database, and upon approval, create new profiles and enters pertinent data in accordance with the local style guide • Index agency services using the Human Services Indexing System (211HSIS), and ensure database is structured in accordance with Inform USA Certification, the local style guide, and industry best practice • Maintain an audit trail for each organization that shows the number of attempts to update, who completed the updates, and when the updates were successfully completed • Document all system updates, changes in contact information, profile merges, and other daily activities for tracking and quality assurance purposes • Assist in development, creation, and implementation of client intake forms • Respect and maintain client confidentiality; follow all agency and program guidelines • Demonstrate a desire to improve with active and open participation in quality assurance program and ongoing process improvement initiatives • Complete required initial and ongoing training as needed • Partner with Data Analyst to improve efficiency, enhance overall performance, and maximize community impact. The position requires a unique combination of analytical skills, technical prowess, and business acumen. • Serve as back-up for the Data Analyst for the following activities: running reports, analyzing data, and responding to internal and external data requests. • Perform other such duties as may be assigned by the senior leadership team • These job responsibilities are to be the main emphasis of this position. Education and Experience
Experience/Qualifications: • 5 years of social service or healthcare networks and/or program management experience or equivalent. • Excellent knowledge of program development, implementation and quality improvement. • Combination of life and work experience and education that demonstrate the abilities to carry out the job functions. • Congruence with agency mission and values. • Experience working in a not-for-profit setting and working with homeless and/or other disadvantaged populations are preferred. • Bachelors and advanced degree in social work, health-care administration or related field. • Previous experience learning and working with new technologies a plus • Ability to pass and maintain a Level 2 background check is required. This is a security background investigation requiring submitting fingerprints to be screened through state law enforcement agencies and the FBI for national criminal history records. • Must pass pre-employment drug screening • Staff members who drive a vehicle on UWNEFL business must provide proof of a valid driver’s license and current automobile insurance. • Must successfully complete Inform USA Certified Resource Specialist- Database curator (CRS-DC) exam within 18 months of eligibility • Proficiency utilizing MS word, MS Excel and database software packages, phones, various software programs and resource files • Experience in database navigation, Community Services or other Information and Referral software experience preferred • Ability to prioritize and quickly switch between projects • Understand, enforce, and adhere to specific regulations as required by accreditation standards, State, and Federal contracts, and agreements • Must have a high attention to detail; concern for accuracy • Self-starter; demonstrated ability to work independently and within teams, meet deadlines and navigate project timelines effectively • Commitment and ability to work with internal and external customers in a friendly, cooperative and professional manner • Flexibility and willingness to embrace change • Proficiency in oral and written communication skills • Comfort with diverse populations is essential • Ability to work effectively under stress and maintain productivity and composure under pressure • Comfortable working in a fast-paced environment • Ability to use good judgment and assessment techniques
