JASMYN supports the empowerment of LGBTQIA+ teens and young adults through leadership, advocacy, resources, and a safe and affirming community. Founded in 1994, JASMYN’s staff of over 25 provides a comprehensive continuum of youth development programs, housing supports, mental health services and health services with a focus on HIV testing, risk reduction counseling, medical linkage for teens & young adults with HIV, and a sexual health clinic that also provides PrEP care coordination for young adults.

We are seeking to hire an experienced Care Coordinator to work primarily with teens and young adults ages 13-29 living with HIV/AIDS (LWHA) and young adults who are vulnerable to HIV who are taking PrEP for HIV prevention ages 18-29 years old. JASMYN care coordinators support young adults and teens LWHA navigating the system of care, while monitoring health outcomes and providing additional safety net services.

Essential duties and responsibilities

• Promote HIV/AIDS health literacy with teens and young adults • Teach & Empower teens and young adults LWHA to manage their HIV Care • Link teens and young adults LWHA to medical care and all other appropriate services • Support teens and young adults LWHA in treatment retention and adherence • Accompany clients to resource meetings and doctor appointments; Provide and/or coordinate transportation services as needed • Link young adults vulnerable to HIV to PrEP and other risk reduction interventions/services • Advocacy, resource brokerage, referral, linkage and follow-up • Coordinate services with other case managers and medical and social service providers, as appropriate • Psychosocial assessment and service planning • Crisis intervention, suicide intervention, and prevention Interagency Collaboration and Partnerships • Coordinate and attend monthly case conference calls with community agencies • Attend monthly Ryan White Co-Op Meetings Outreach • Attend on and off JASMYN Campus health fairs and other outreach events • Support with outreach to teens and young adults vulnerable to HIV • Support with outreach to teens and young adults who have fallen out of HIV care HIV Testing & Sexual Health Clinic • Conduct daily rapid HIV testing, including pre- and post-test counseling both on and off JASMYN Campus, as assigned • Link young adults at risk for HIV to Pre-Exposure Prophylaxis (PrEP) on and off JASMYN campus • Advocacy, resource brokerage, referral, linkage and follow-up • Work JASMYN sexual health clinic, as assigned • Support teen and young adult sexual health education and HIV prevention activities at JASMYN • Support young adults at risk for HIV in applying for Medical Assistance Program (MAP) for PrEP medication Safe Space & Youth Engagement Programming • Contribute to and support JASMYN as a safe and engaging social space for BIPOC LGBTQIA+ teens and young adults • Assist in the facilitation of a Community Advisory Board that will inform and provide input into safe space programming for BIPOC LGBTQIA+ young adults • Assist in Programs for Young Adults Living with HIV • Assist in the offsite retreat for Young Adults Living with HIV Data Collection & Reporting • Proficient in Microsoft Office, particularly Excel • Data entry • Document and report all services according to all requirements as required by JASMYN polices and standards within 48 hours • Conduct monthly data reviews Additional Essential Duties • Participate as a member of the JASMYN team in supporting all JASMYN programs and services for high risk youth, including group facilitation, outreach and enrollment, drop in center and clinic staffing • Adherence to HIPAA law to ensure participant confidentiality and privacy at all times. • Participate in all meetings and trainings as required • Perform other duties as assigned

Qualifications

• Multilingual in English, Spanish, and/or Portuguese is a plus • Ability to relate to teens and young adults in an accepting, positive and healthy manner; Demonstrated experience in positive youth development. • Familiarity and sensitivity in dealing with the unique needs of LGBTQIA+ teens and young adults. • Experience and/or knowledge with HIV prevention, testing and treatment services • Ability to work with a diverse staff and excel in a multi-cultural environment. • Ability to effectively deliver strength-based, trauma-informed, and harm reduction services. • Ability to coordinate multiple responsibilities and prepare regular reports • Ability to develop and lead group and individual sessions with youth that have multiple needs. • Strong assessment skills and ability to document and prioritize needs; Ability to facilitate case planning, resource brokerage and referral • Demonstrated computer and keyboard proficiency using Microsoft Office software (Word & Excel) • Demonstrated ability to work independently with accountability; exercise sound judgment, discretion and professionalism at all times • Demonstrated ability to take initiative in preforming all tasks and duties • Excellent written and verbal communication skills and attention to detail • Desire to make a positive impact in the lives of LGBTQIA+ youth and to be a part of a dynamic team supporting LGBTQIA+ youth • Current FL driver’s license and clean driving record; Must pass criminal background checks

Education and Experience

BA or Master’s Degree in Social work or a related field such as public health, social welfare or psychology from an accredited university acceptable, with a minimum of three years of youth case management or similar experience. Certified in HIV testing and counseling preferred. Training in evidence-based interventions for HIV positive youth is a plus!

Salary/Salary Range

$19/hr
