The Catholic Charities Jacksonville Regional Office is seeking a full-time HSPRS Case Manager. This position is located in Jacksonville, FL. The Safe Passages program assists unaccompanied Refugee children with their transition into the United States. Under the U.S. Office of Refugee Resettlement CCB provides Home Study and Post Release (HSPRS) services to unaccompanied children. Through this program, unaccompanied children and their sponsors receive a variety of services designed to facilitate integration and empowerment among immigrant families in their new communities. Tens of thousands of unaccompanied children seek refuge in the United States each year. Most are fleeing threats and violence in El Salvador, Guatemala, or Honduras. The children are released from the Office of Refugee Resettlement custody into the care of a sponsor. Most (85%) join a relative or friend already living in the United States. It is often a tough transition. They adjust to new communities, cultures, languages, and family units. Each child has mental health, safety, education, and legal needs that must be met. Our staff work with these children and their sponsors. In the Home Study part of the program, we will visit the homes where children will be living before, they get there. We will interview each member of the household and ensure this is a safe placement for the child while supporting the sponsor to prepare for the responsibility of promoting the arriving child’s safety, rights, and needs. Once the child is living with their sponsor, the Post Release Services component of our work starts. CCB staff will work with the children and sponsors to create an action plan. They will set goals like finishing school, obtaining legal immigration status, or connecting to other community support systems. Then we will check in with the child and their sponsors at least once a month to make sure the child is safe and making progress. The entire process will take place in whatever language the child and their sponsor are most comfortable speaking.
Position Summary: The Safe Passages Case Manager within the HSPRS Program is responsible for overseeing and managing the comprehensive support services provided to individuals and families. This role involves assessing client needs, developing service plans, and collaborating with internal and external stakeholders to ensure the success of the program participants. The Family Reunification Program provides case management through Home Studies and Post-Released Services to unaccompanied minors in immigration proceedings.
Essential duties and responsibilities
Conduct thorough assessments to identify clients' strengths, needs, and goals. Collaborate with clients to develop personalized service plans. Coordinate and monitor the delivery of services to meet clients' identified needs. Provide crisis intervention and supportive counseling as required. Advocate for clients to ensure access to appropriate resources and services. Empower clients to make informed decisions about their goals and aspirations. Collaborate with internal team members and external service providers to address clients' complex needs. Attend meetings with community partners to enhance collaboration and resource sharing. Maintain accurate and up-to-date client records in compliance with program standards. Generate reports as required and contribute to program evaluations. Respond to crisis situations promptly, providing necessary support and resources. Work closely with clients to prevent and address crises effectively. The Home Study and Post-Release Case Manager (HS/PRS- CM) will conduct home visits and phone calls to evaluate the safety and suitability of the minor's placement with their caregiver and assess/address the minor's needs. Provide ongoing case management focusing on screening and making appropriate referrals to address the minor's needs related to placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, and child enrichment activities. Complete and document all required reports, case notes, and contacts in the appropriate database by the expected timeframes and requirements. Collaborate with CBOs, law enforcement agencies, or government offices to ensure the safety and well-being of the minor. Provides crisis intervention services as needed. Participate in outreach and networking events in the community to try to leverage more resources for the families. Assist in other special projects for the family reunification program as needed. Other Duties and Functions: Develop an atmosphere and work processes which treat all clients with dignity and respect. Attends all Agency, departmental, and partnership meetings as required. Consistently demonstrate a positive, friendly, respectful, and caring attitude with staff, clients, volunteers, and visitors, projecting a welcoming atmosphere to all. Organize and prioritize work, be proactive, take initiative, resolve complex problems, and simultaneously manage multiple priorities. Embrace the philosophy of creating a nurturing culture in support of staff development. Adhere to agency standards of ethical conduct and maintain professional boundaries with staff, volunteers, and clients. Adhere to agency standards involving the handling of highly confidential and sensitive information. Display sensitivity to the served population’s cultural and socioeconomic characteristics. Participate in in-service activities, training, continuing education, conferences and workshops for professional development and compliance with grant and Agency policies. Keep detailed records per Agency and department guidance. Correctly enter required information in the prescribed databases, in a timely manner. Comply with regulatory, statutory, contractual, grant and Agency standards for timeliness, completeness and accuracy when preparing and maintaining paperwork and required reports. Comply with Diocesan and Catholic Charities Bureau, Inc. general policies, regulations, and procedures.
Qualifications
Bachelor's degree in social work, psychology, counseling, or a related field; Master's degree is a plus. Experience in social work settings preferred. Experience with case management. Fully bi-lingual in both English and Spanish, preferred.
Education and Experience
Clear Level II background screening. Clear a reference check (professional and personal) Clear a local background check from the county in which you reside. Clear an annual Motor Vehicles Records check. Provide proof of and maintain a minimum personal auto insurance according to agency standards. Provide a copy of all certifications prior to your first day of employment. Clear an E-Verify check. Valid Florida state driver’s license and a reliable car. Driving and Travel 40% travel is required for this job. Overnight travel may be required for this position. This position requires that you drive your personal automobile or vehicle on agency business on behalf of Catholic Charities Bureau, Inc. Therefore, you must be at least 25 years old and have a current valid Florida Driver’s License with no more than 5 points on your Florida driver’s license within the last three years and no DUI convictions. You are required to carry automobile liability insurance in the amount of at least $100,000 for each person, $300,000 for each accident or occurrence for bodily injury, and $50,000 for property damage.
